Our caring and friendly office staff is here to serve you.

In order for us to provide you with the best individual care, we request that you schedule an appointment in advance. Scheduling in advance will enable our staff to properly prepare for your visit, as well as provide you with more time with Dr. Miller.

Our office is conveniently located on 5th Avenue in St. Petersburg, Florida. We are on the north side of 5th Avenue, at 5th Avenue and 42nd Street. Our ample parking lot is located in the front of the office.

Confidentiality

Our caring staff recognizes and respects our patient's privacy, and we guard it. Your medical records are kept in strictest confidence. We will not provide information from your records to anyone without your express written consent.

Financial Policy

Thank you for choosing us as your health care provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered part of your treatment.

Insurance Coverage: We accept assignment on most insurance plans. However, we do not require that co-pays, deductibles, and percentages be paid at the time of service. Please be aware that a few select services are considered non-covered and/or not considered reasonable and necessary under Medicare programs and some medical insurance plans. These services would be your responsibility.

Non-Insurance/Self Pay: Full payment is due at the time of service. We accept cash, checks or credit cards, and offer an extended payment plan with prior credit approval.
